Christiaan Barnard

Christiaan Neethling Barnard (8 November 1922 - 2 September 2001) was a South African cardiac surgeon who, on 3 December 1967, performed the first human-to-human heart transplant at Groote Schuur Hospital in Cape Town, South Africa.[1][2] The recipient, Louis Washkansky, a 54-year-old man with end-stage heart disease, received the heart of Denise Darvall, a young woman fatally injured in a road accident; Washkansky regained consciousness and survived for 18 days before dying of pneumonia.[1][2] The operation was an international sensation and made Barnard one of the most famous physicians of the twentieth century.[2][3]

The first human heart transplant

Barnard described the operation in a primary report published just weeks later in the South African Medical Journal: Barnard CN, "The operation. A human cardiac transplant: an interim report of a successful operation performed at Groote Schuur Hospital, Cape Town," S Afr Med J. 1967;41(48):1271-1274 (PMID 4170370).[1] The procedure built on a decade of experimental cardiac surgery and on techniques for cardiopulmonary bypass and orthotopic heart transplantation developed in the United States, particularly the experimental work of Norman Shumway and Richard Lower.[2][3] The donor, Denise Darvall, was declared dead after a severe head injury, and her heart was transplanted into Washkansky once it had stopped, a sequence reflecting the legal and ethical understanding of death at the time and predating modern brain-death criteria.[2][3] See the dead donor rule and Uniform Determination of Death Act and brain death for how death determination later evolved.

Washkansky's death after 18 days from pneumonia was attributed in part to the immunosuppressive drugs used to prevent rejection, which left him vulnerable to infection, illustrating the central dilemma of transplantation before the advent of selective immunosuppression.[1][2] Barnard's second heart transplant patient, Philip Blaiberg, operated on in January 1968, survived for about 19 months, demonstrating that meaningful survival was achievable.[2][3]

Aftermath and the early heart-transplant era

Barnard's operation set off a wave of heart transplants worldwide in 1968, but early results were poor, and within a few years most programs had stopped because patients were dying of rejection and infection.[2][3] Heart transplantation did not become a durable, standard therapy until the development of better immunosuppression, above all ciclosporin in the late 1970s and early 1980s, and improved methods of diagnosing and treating rejection.[3][4] See heart transplantation for the modern procedure.

Later life

Barnard continued to operate and to publish, and he became a public celebrity, a status that drew both admiration and criticism within the medical profession.[2] Progressive rheumatoid arthritis in his hands eventually ended his surgical career in 1983.[2] He died of an asthma attack in 2001.[2] Historical accounts also note the contributions of the broader Groote Schuur team, including his brother Marius Barnard and others, and the role of Hamilton Naki, a laboratory assistant whose contribution to the surgical research program was acknowledged more fully in later years.[2][3]

Significance

The 1967 operation proved that human heart transplantation was technically possible and captured global attention, accelerating both scientific and ethical engagement with transplantation, including debate over the determination of death, which contributed to the development of brain-death criteria in the years that followed.[2][3] Although the procedure's clinical promise was not realized until immunosuppression improved, Barnard's transplant is a defining milestone in the history of organ transplantation.[2][3]
